Job description

Position Overview:
Seeking a dedicated Registered Nurse to provide high-quality, patient-centered care in a home health setting. This role involves delivering skilled nursing care to patients in their homes, supporting recovery, managing chronic conditions, and promoting overall wellness.
Schedule & Work Details:
  • Schedule: Monday through Friday
  • Weekly Hours Guarantee: 32 to 36 hours
  • On-Call/Weekend Requirement: Yes
  • Coverage Area: Fort Wayne, IN and surrounding areas
  • Mileage Reimbursement: Yes, reimbursed between patient visits from first to last patient
  • Pay Rate: $51 to $53 per hour
Key Responsibilities:
  • Provide direct skilled nursing care to home health patients in accordance with physician orders and care plans
  • Perform comprehensive patient assessments and ongoing evaluations
  • Administer medications, treatments, and interventions safely and effectively
  • Monitor patient progress and adjust care plans as needed
  • Educate patients and caregivers on medical conditions, medications, and home care best practices
  • Maintain accurate and timely clinical documentation
  • Collaborate with interdisciplinary team members to ensure coordinated patient care
  • Participate in on-call rotation and respond to patient needs as required
Required Qualifications:
  • Active Registered Nurse license
  • Ability to provide patient care in a home health environment
  • Strong clinical assessment and documentation skills
  • Valid driver's license and reliable transportation
Preferred Qualifications:
  • Previous home health experience
  • Familiarity with home health documentation and workflow
  • Strong communication and patient education skills
