• Conduct comprehensive psychosocial assessments to evaluate clients’ mental health challenges, strengths, and support systems.
• Provide individual, family, and group therapy sessions utilizing evidence-based practices such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care.
• Develop, implement, and monitor individualized treatment plans that reflect clients' specific goals and interventions, ensuring alignment with their emotional and psychological needs.
• Collaborate with an interdisciplinary team of psychiatrists, psychologists, and healthcare professionals to create a coordinated care approach for clients.
• Facilitate group therapy sessions, psychoeducation classes, and support groups to enhance community and emotional resilience among clients.
• Provide crisis intervention, risk assessments, and safety planning when necessary to ensure client safety and well-being.
• Maintain accurate, timely documentation of all client interactions and treatment plans, adhering to ethical and legal standards.

• Master's degree in Social Work from an accredited institution.
• Active and unrestricted state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong knowledge of evidence-based therapeutic approaches, with an emphasis on trauma-informed care.
• Excellent interpersonal, communication, and assessment skills applicable in a remote setting.

• A minimum of two years of clinical experience in a mental health or outpatient setting.
• Previous experience working with hospice patients or knowledge of palliative care practices is highly desirable.
• Familiarity with remote therapy platforms and digital documentation tools.
