Job Summary:

Carease Health - MD, a nationwide preventative healthcare company, is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. As a Nurse Practitioner, you will play a pivotal role in our comprehensive preventative care program, incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to minimize hospitalizations and elevate patient care standards.

Responsibilities:

  • Deliver high-quality, patient-centered care to residents in skilled nursing facilities through on-site visits and remote work from home

  • Collaborate with our dedicated scribe team for efficient documentation and implementation of patient care plans

  • Build strong relationships with facility staff, patients, and families to ensure comprehensive care

  • Participate in ongoing training and education to stay current with the latest clinical practices and guidelines

  • Maintain accurate and timely records of patient care, including remote patient monitoring and chronic care management

Requirements:
