Job description

FULLY Remote position- Must live in EST or CST as you will be working 8-4:00 p.m. on Eastern Standard Time.

Responsible for delivering a structured, evidence-based diabetes education and coaching program for members with diabetes. The role is focused on improving self-management, medication adherence, lifestyle modification and clinical outcomes for a diverse member population.
This position will provide assessment, health education and behavior change counseling using a collaborative approach to facilitate change.

EXPERIENCE
Two (2) years of experience in a health promotion/education setting, counseling on health topics such as nutrition, exercise, cholesterol, hypertension, stress, alcohol and smoking.

One- Two years of experience in delivering chronic condition coaching or diabetes education.
Health & Welllness education experience conducting assessment to determine needs through telephonic outreach and health risk appraisals.

EDUCATION

  • Bachelor’s degree in health education, Health Promotion, Nutrition, Nursing, Exercise Physiology or Social Work required

Relevant combination of education and experience may be considered in lieu of degree

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:

Provide coaching for diabetes condition management including self-management, glucose monitoring, medication use, nutrition, physical activity, stress reduction, reducing complications, etc.

Call and coach members on healthy habits and disease prevention.

Help members manage diabetes through education on medication, nutrition, exercise, glucose monitoring, and lifestyle changes.

Use motivational interviewing and goal-setting techniques to encourage behavior change.

Connect members with healthcare providers and community resources when needed.

Document member interactions and track progress.

Support wellness and prevention programs focused on improving member outcomes.

Knowledge of NCQA, CMS, URAC, HEDIS standards as applicable to designated department assignment.
