Care Coordinator

Frame is seeking a Care Coordinator with strong experience in supporting patients during preconception and fertility treatment.

About the Role

We are currently hiring a Care Coordinator to join our fully remote team. This is a unique, flexible position with a minimum of 20 hours per week and the potential to increase over time and/or convert to hire in a PT/FT position. In this role, you will:

  • Empower individuals and couples to reach their family goals and help them understand the next steps on their journey.
  • Effectively communicate to patients asynchronously, utilizing Frame tools and processes.
  • Follow up on clinical recommendations and care plans that are personalized to each patient and their family goals.
  • Collaborate with other members of the care team (Medical Director, Health Coaches, Nurse Practitioners) to ensure comprehensive support for patients at all stages of their journey.
What We're Looking For
  • You love a flexible schedule and have the ability to work at least 20 hours/week (ideally mornings Eastern/Central Time), and more when needed.
  • Empathetic and non-judgmental. Fertility journeys can have their challenges and we're here to support our patients.
  • Exceptionally detail oriented and organized, with excellent written and verbal communication skills.
  • Experience supporting patients in telemedicine / virtual care setting, especially those in women's health or fertility
Requirements
  • Experience working in healthcare is required
  • 2+ years of experience with fertility, reproductive health, and/or women's health care
  • B.S. Degree from an accredited institution.
  • An RN or MA license is a bonus but not required
  • Startup experience a plus!
