Redefine patient engagement through personalized, remote MTM services.

Key Responsibilities:

  • Conduct telephonic or video MTM sessions for Medicare and commercial patients.
  • Document clinical interventions and CMRs in compliance with CMS standards.
  • Identify and resolve medication-related problems and adherence barriers.

Qualifications:

  • PharmD and MTM certification preferred.
  • Prior MTM, chronic disease management, or ambulatory care experience.
  • Ability to connect and build trust with patients virtually.
