As a part of our hospice center, you will frequently communicate with colleagues via virtual platforms to coordinate care and share insights, ensuring a collaborative approach to client wellbeing. This unique setting allows for a blend of clinical precision and compassionate care, tailored to the needs of individuals dealing with end-of-life issues.

• Provide individual and group therapy for clients exhibiting signs of severe mental illness and emotional disturbances, utilizing evidence-based therapeutic techniques tailored to the hospice context.
• Conduct thorough assessments to identify client needs and develop individualized care plans that encompass both psychological and practical support.
• Collaborate closely with other healthcare professionals to ensure a holistic approach to client care, sharing critical information and resources effectively.
• Engage actively as a member of the interdisciplinary team, contributing insights and feedback while working toward common goals for client outcomes.
• Facilitate crisis intervention when necessary, employing de-escalation strategies to maintain a safe environment for clients and staff.
• Document client interactions promptly and accurately, ensuring compliance with organizational policies and ethical standards.
• Participate in ongoing training and development opportunities to enhance your skills and stay abreast of best practices in the field.
• Provide on-call support, responding to any urgent concerns that may arise outside regular hours.

The primary focus of this role is to support patients within the hospice setting who are dealing with end-of-life issues, including severe mental health conditions that may be exacerbated by terminal illnesses. Our clients often come from diverse backgrounds, and it is essential to approach each individual with cultural competence and sensitivity to their unique experiences. Your role will include performing therapeutic assessments, recognizing the intersection of mental health and chronic illness, and providing targeted interventions that support both emotional and practical needs.

• A valid and active Licensed Clinical Social Worker (LCSW) credential in the State of Oregon.
• A Bachelor’s Degree in Social Work or a related field; a Master's degree is preferred.
• Minimum of 2 years of professional experience working in clinical settings, particularly with mental health and hospice clients.
• Expert knowledge of assessment techniques and therapeutic interventions.
• Strong communication skills and the ability to build rapport with clients and their families.

• Experience working in hospice or palliative care settings.
• Familiarity with telehealth practices and remote service delivery.
• Experience collaborating within interdisciplinary teams to create comprehensive care strategies.

Community Outreach Mental Care offers a competitive salary range of USD 80,000 to 100,000 per year, commensurate with experience.
