This position is fully remote, offering you the flexibility to work from the comfort of your home while collaborating with a supportive team of healthcare professionals. You will regularly engage in video conferencing and digital communication to coordinate care, ensuring that our patients receive comprehensive social support. • Conduct thorough biopsychosocial assessments to understand the holistic needs of patients and their families.
• Develop individualized, evidence-based treatment plans that address specific psychosocial needs and enhance patient care.
• Implement interventions aimed at improving the mental health and emotional well-being of patients facing terminal illnesses.
• Maintain accurate clinical records in compliance with confidentiality standards and regulatory requirements.
• Collaborate closely with interdisciplinary teams, including physicians, nurses, and chaplains, to create a seamless care experience.
• Serve as an educator for patients and families regarding mental health issues, coping strategies, and available resources.
• Monitor and evaluate treatment efficacy, adjusting plans as necessary to meet changing patient needs.
• Stay informed about the latest developments in mental health care, ensuring that your practices remain current and effective.

In this role, you will work primarily with hospice patients suffering from terminal illnesses, including cancer, heart disease, and neurodegenerative conditions, as well as their families. Your expertise will be critical in facilitating conversations around end-of-life care, grief, and emotional distress, ensuring that both patients and their loved ones navigate this journey with support and dignity.

• A Master’s degree in Social Work from an accredited institution.
• A valid Licensed Clinical Social Worker (LCSW) license in the state of Georgia.
• At least two years of clinical experience, ideally in a hospice, palliative care, or mental health setting.
• Proficiency in solution-focused and trauma-informed psychotherapy interventions.
• Strong ability to communicate and connect with diverse populations, demonstrating empathy and understanding.
• Excellent problem-solving skills and the ability to think critically in high-pressure situations.
• A commitment to working collaboratively within a multidisciplinary team.

Experience within hospice care or a similar healthcare setting is preferred, as this enables you to quickly adapt to the unique challenges and rewards of working with terminally ill patients. Familiarity with electronic health records (EHR) systems will also enhance your effectiveness in this role, allowing you to keep accurate and efficient documentation of patient interactions.

We offer a competitive salary range of USD 80,000 to 100,000 annually, reflecting your skills and experience.
