Key Responsibilities

In this role, you will be responsible for the following:

  • Act as an advocate for patients and their families, facilitating effective communication with relevant parties concerning treatment plans and available resources.
  • Identify patients at risk for avoidable delays or complex discharge needs and collaborate with care providers to find optimal solutions.
