• Conduct comprehensive initial assessments to fully understand patients’ mental health backgrounds and needs.
• Develop and implement tailored treatment plans based on evidence-based practices to address individual challenges.
• Provide high-quality individual and group psychotherapy sessions, utilizing various therapeutic modalities appropriate for specific patient needs.
• Conduct crisis interventions in real-time to assist patients through acute psychological distress.
• Maintain accurate and detailed documentation of patient progress in compliance with legal and ethical standards, leveraging electronic medical record systems.
• Collaborate proactively with a multidisciplinary team, including physicians, nurses, and other healthcare professionals, to coordinate comprehensive patient care.
• Engage in regular clinical supervision and participate in team meetings to enhance skills and share insights that benefit patient outcomes.
• Stay updated on the latest research and methodologies in mental health care to continually improve treatment approaches.

Your expertise will serve a varied patient population including those dealing with chronic illness, grief, and emotional distress within the hospice context. You will work closely with individuals from diverse backgrounds, displaying cultural competence and sensitivity to ensure all patients feel heard and understood. Your role is pivotal in helping patients navigate complex emotional landscapes and establish coping mechanisms that enhance their quality of life.

• A Master’s degree in Social Work from an accredited program, equipping you with the theoretical knowledge and practical skills necessary for this role.
• A valid and active LCSW license to practice clinical social work in New Jersey.
• Proven experience delivering psychotherapy to a diverse client group, with a strong emphasis on adaptability in therapeutic approaches.
• Exceptional communication, active listening, and interpersonal skills to effectively connect with both patients and colleagues.
• Familiarity and proficiency in electronic medical records and telehealth technologies that facilitate modern healthcare practices.

• Experience in hospice care or working with terminally ill patients is highly desirable.
• Previous involvement in multidisciplinary team settings that emphasize collaborative care.
• Knowledge of evidence-based therapies specific to grief and bereavement care, as well as trauma-informed care principles.

Clear Meadows Counseling Center offers a competitive salary ranging from USD 80,000 to 100,000 annually, based on experience and qualifications.
