Job Title: Registered Nurse (RN) Case Manager โ€“ Home Health | Dedicated Northwest Territory + 1:1 Patient Care

Company: Home Health Care, Inc.

Territory: Maple Grove, Dayton, Elk River, Rogers, Osseo, Otsego, Albertville, Hanover & St. Michael + approximately 35 miles from your home for on-call and weekend coverage

Schedule: Full-Time | Mondayโ€“Friday, 8:00 AM โ€“ 5:00 PM | 1 Weekend Per Month | 1 On-Call Shift Per Week

Compensation: $95,000 โ€“ $130,000 annually plus incentive visit opportunities and $6,000 in first-year bonuses

What You'll Do

  • Manage a home health caseload throughout Maple Grove, Dayton, Elk River, Rogers, Osseo, Otsego, Albertville, Hanover, and St. Michael.
  • Complete OASIS assessments and individualized plans of care.
  • Deliver skilled nursing visits and patient education.
  • Coordinate care with physicians and interdisciplinary team members.
  • Help patients remain healthy, independent, and safely at home.
  • Maintain timely and accurate clinical documentation.

What We're Looking For

  • Active Minnesota Registered Nurse (RN) license.
  • Strong clinical judgment and ability to work independently.
  • Home health experience preferred, but not required.
