• Conduct comprehensive clinical assessments and formulate individualized treatment plans tailored to the unique emotional and psychological needs of hospice patients and their families.
• Deliver therapeutic interventions including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) through secure telehealth platforms, ensuring all interactions are confidential and effective.
• Maintain precise and comprehensive clinical records, adhering to all regulations and standards, while efficiently utilizing electronic health record (EHR) systems to document patient progress and treatment outcomes.
• Engage in virtual consultations with multidisciplinary teams, including physicians and other mental health professionals, to coordinate care and discuss treatment approaches for complex cases.

• Educate patients and their families about mental health issues and available support resources, guiding them in navigating the complexities of hospice care through digital tools and resources.
• Continuously update your knowledge of mental health best practices through virtual workshops and online resources, integrating the latest evidence-based methods into your practice.

Your primary focus will be on delivering compassionate care to hospice patients who may be grappling with mental health challenges related to their conditions. By providing empathetic support and treatment, you will improve their quality of life during a sensitive period. Engaging with family members will also be essential in offering them the tools they need to support their loved ones effectively.

• A Master's degree in Social Work from a recognized institution, emphasizing therapeutic techniques applicable in hospice and palliative settings.
• A valid LCSW license to practice in California, demonstrating your qualifications and adherence to professional standards.
• A minimum of 2 years of clinical experience, ideally with direct service delivery to patients in either hospice, palliative care, or community health settings.
• Proficient in solution-focused psychotherapy techniques, especially those that can be effectively delivered through remote channels.
• Strong communication skills, enabling you to build rapport with patients through virtual interactions, trusting relationships, and effective treatment outcomes.
• Demonstrated ability to solve problems creatively and work independently while also functioning effectively within a remote team.
