This role will be conducted primarily in a remote setting, which allows for flexibility while you work closely with clients in their hospice journey. Our team effectively utilizes digital platforms for video consultations and case management, ensuring that our services remain seamless and responsive. You will be well-supported by our administrative staff and have access to the resources necessary to perform your role efficiently, even from a distance.

• Conduct comprehensive assessments to understand the psychological, social, and environmental factors impacting clients’ well-being.
• Develop individualized treatment plans that align with each client’s goals and values, while employing evidence-based interventions.
• Provide therapeutic sessions via telehealth to support clients in managing their mental health and coping strategies.
• Maintain detailed and accurate clinical documentation, adhering to compliance standards and recognizing the importance of confidentiality.
• Collaborate with a multidisciplinary team, including nurses, physicians, and spiritual care counselors, to coordinate integrated care for clients.
• Educate clients and their families about mental health management, available resources, and coping techniques related to end-of-life care.
• Stay current with trends in mental health practices, ensuring your methods remain relevant and effective.

This position primarily serves clients in a hospice center, which means you will be working with patients facing terminal illnesses. Your expertise will be vital in providing emotional support not only to the patients but also to their families, helping them navigate grief, acceptance, and the complexities surrounding end-of-life decisions. As an LCSW, your approach will emphasize compassionate engagement, ensuring that care is tailored to the unique needs and circumstances of each individual you encounter.

• A Master’s degree in Social Work from an accredited institution is required.
• Must possess a valid LCSW license in good standing.
• Demonstrated experience in clinical assessments, treatment planning, and implementation of therapeutic interventions.
• Excellent interpersonal skills and the ability to communicate effectively with patients, families, and colleagues.
• Proficiency in using remote technology platforms for consultations and documentation.

• Minimum of two years of experience in a clinical social worker role, particularly within hospice, palliative, or end-of-life settings.
• Familiarity with trauma-informed care practices and evidence-based therapeutic techniques.
• Experience collaborating within interdisciplinary teams to deliver high-quality patient care.
