Horizon Mental Care Services is seeking a dedicated and experienced licensed Clinical Social Worker (LCSW) to join our remote team in providing exceptional mental health care. In this full-time role, the LCSW will be responsible for leading psychoeducational and process-oriented therapy groups, conducting individual therapy sessions, performing clinical assessments, implementing effective interventions, and managing various case management responsibilities.

• Initiate discharge planning from the very first day of admission for all patients, proactively addressing their needs and preparing them for a successful transition.
• Conduct comprehensive discharge sessions with patients to ensure they understand and have developed vital coping skills, a clear transition plan, and knowledge of available resources.
• Confirm discharge appointments with patients and their loved ones ahead of time, ensuring all parties are prepared for the upcoming changes.
• Communicate with referral sources and outpatient providers at key points—upon admission, mid-treatment, and pre-discharge—to guarantee continuity of care and seamless transitions for patients.
• Facilitate patient transfers to higher levels of care when necessary, ensuring that each patient receives the appropriate level of support.

You will be working primarily with adults experiencing a wide range of mental health conditions, including anxiety, depression, trauma-related disorders, and more. Your therapeutic approach will be patient-centered, focusing on building therapeutic rapport and fostering resilience. This role is critical in addressing social determinants of health and promoting holistic well-being, as our patient population often faces various external challenges.

• A Master’s degree in Social Work from an accredited institution.
• Active licensure as a Clinical Social Worker (LCSW) in the State of California is required.
• Exceptional written and verbal communication skills to effectively engage with patients, families, and healthcare providers.
• Demonstrated ability to maintain professional boundaries and provide a stable therapeutic environment for clients.
• Proficiency in completing and submitting clinical documentation promptly and per regulatory standards.

• Experience in a community health clinic setting or working with diverse populations is preferred.
• Familiarity with telehealth platforms and digital mental health tools is advantageous.
• Previous experience in case management and interdisciplinary collaboration enhances the quality of patient care.

This position offers a competitive annual salary ranging from $120,000 to $130,000, depending on your experience and caseload.
