Job description

We are seeking an RN Case Manager/Monthly Assessment Nurse to join our team! You will be responsible for the assessment, care plans, and documentation of assigned residents.

Position: Assessment RN / Case Manager
Location: Sonoma County
Hybrid Position, Remote and on-site monthly visits

Conduct initial and monthly assessments on residents in assisted living facilities/Skilled Nursing facilities

Pay rate: $75/per assessment (20 mins)

This can be a part-time or full-time position, you may accept up to 60 residents per your caseload, work on your own time.

Responsibilities:

  • Visit- residents at a local assisted living facility, monthly
  • Monthly assessment, semi-annual comprehensive assessment with a care plan
  • Monitor and report changes in patient symptoms or behavior.
  • Communicate with collaborating physicians, administrators, Home Health agency, Hospice agency, POA, family members regarding patient care.
  • Educate patients about health maintenance and disease prevention
  • Facilitate referrals to other healthcare professionals and medical facilities
  • Maintain accurate patient medical records
  • Provide advice and emotional support to residents and their family members
  • You are in charge of your schedule, flexibility is welcome.

Qualifications:

  • Previous experience in nursing and/or case management fields
  • Familiarity with medical software, office suites/excel, word
  • Ability to build rapport with residents and referral sources
  • Strong problem solving, confidence in your nursing abilities

Requirements

  • Must have an active RN-nursing license in the state of CA- in good standing
  • Experience in Home Health, Geriatric, Long-term Care a plus
  • Current BLS
  • Current TB Skin test
  • Must have experience with office suites - Excel/Word/PDF
  • Two current references
  • Must be able to pass a background screening
