Description:

American Hospice and Home Health Services is currently seeking a Full-Time or Part-Time Speech Therapist / Speech-Language Pathologist (SLP) to service any combination of these counties:

  • Alameda County
  • Contra Costa County
  • Solano County
  • San Joaquin County
  • Stanislaus County

We offer per-visit pay in addition to hourly or salary options depending on experience.

Per Visit Rates: $120 $200 per skilled speech therapy visit
Hourly Option: $50$65 (Office hours)

The Speech Therapist is responsible for evaluating, planning, implementing, and monitoring speech, language, swallowing, and cognitive therapy services for home health patients. You will coordinate with nursing, physicians, caregivers, and other therapy staff to deliver high-quality, patient-centered care.

Requirements:

Essential Job Functions / Responsibilities

Evaluation & Assessment

  • Perform comprehensive speech, language, voice, swallowing, and cognition assessments in the patients home.
  • Review medical records, labs, diagnostics, and collaborate with nursing to incorporate patient history.
  • Evaluate the home environment to support therapy success (safety, assistive devices, caregiver support).
  • Identify patient, family, and caregiver needs, preferences, and goals.

Treatment Planning & Implementation

  • Develop individualized, measurable therapy plans aligned with patient goals, family input, and physician orders.
  • Provide direct skilled therapy (speech, language, swallowing, cognitive) to patients.
  • Adjust therapy plans based on patient response and progress.
  • Instruct and train caregivers on carryover strategies and home exercises.
  • Coordinate with an interdisciplinary team (nursing, OT, PT, CNAs) to align therapy with overall care plan using google workspace applications and EMR system, known as HomeHealthMD .

Documentation & Compliance

  • Complete evaluations, treatment notes, progress notes, re-evaluations, and discharge documentation per agency policy and regulatory timelines (48 hours).
  • Document all therapy contacts, missed sessions, cancellations, and outcomes accurately.
  • Record therapy elements that support billing, justification, and compliance.
  • Communicate changes in patient status or plan to physicians and care team.

Communication & Collaboration

  • Participate in care team meetings, case discussions, and interdisciplinary conferences.
  • Report therapeutic findings, recommendations, and concerns to physicians, nursing, and leadership.
  • Provide clear, respectful, and timely communication with patients, families, caregivers, and staff.

Additional Duties

  • May cover on-call therapy as needed according to agency policy if both parties agree.
  • Assist in scheduling, travel coordination, and therapy resource management.
  • Maintain continuing education and fulfill licensure / competency requirements.

Licenses & Requirements

  • Valid Speech-Language Pathologist license in California
  • Minimum 1 year of experience in home health therapy, outpatient, hospital, or similar setting preferred
  • Certification with ASHA (CCC-SLP) or relevant credential
  • Current CPR / BLS certification
  • Reliable vehicle, valid drivers license, and appropriate insurance
  • Strong communication, organizational, and documentation skills

Work Location & Travel

  • Home visits across the counties that the therapist chooses.
  • Must be able to travel among patient homes
  • Flexible scheduling (full-time or part-time options; remote documentation work between visits)

Compensation details: 50-65 Hourly Wage
