• Assist patients in accessing community resources, such as support groups, vocational rehabilitation programs, and housing assistance, facilitating their reintegration into the community.

• Educate patients and families about available community resources and post-rehabilitation services, empowering them with information to support their recovery process.

In your role as a Licensed Clinical Social Worker, you will focus on a diverse patient population that includes individuals recovering from conditions such as stroke, spinal cord injuries, orthopedic surgeries, and neurological disorders. • A Master's degree in Social Work from an accredited program is required.
