Work Environment

This role is designed for a remote work setting, allowing for flexibility while maintaining close collaboration with our multidisciplinary team. Key Responsibilities
  • Serve as a primary contact for clients, performing comprehensive assessments and developing personalized treatment plans that are aligned with evidence-based practices.
  • Communicate regularly and effectively with physicians, reporting on patient progress, treatment needs, and any changes in patient condition that may arise.
  • Consult and coordinate with families and support systems to ensure comprehensive treatment and discharge planning that considers the unique needs of each client.
  • Document all clinical interactions, maintaining accurate and timely records of clinical notes and progress updates in compliance with departmental policies and applicable legal standards.
  • Work collaboratively with a healthcare team, engaging in the development of discharge plans based on psychosocial assessments and agreed-upon interventions.
  • Facilitate care coordination by connecting clients with regional service providers, assisting with access to required services and resources throughout their recovery journey.
Patient Population / Client Focus

The ideal candidate will work with a diverse patient population experiencing various mental health issues, including anxiety, depression, and substance use disorders. Our clients range from adolescents to adults, and your expertise will be pivotal in providing them with tailored therapeutic support that fosters recovery and personal growth.

Qualifications
  • A Master’s degree in Social Work, Counseling, Marriage and Family Therapy, or Psychology from an accredited institution.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC) or equivalent.
  • Minimum of three years of clinical experience in an outpatient setting, demonstrating a robust understanding of mental health treatment protocols.

  • Strong organizational skills and the ability to work both independently and as part of a cohesive team.
Preferred Experience
