Mindfulbear Counseling Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in providing essential mental health services remotely for clients in hospice care. The social worker will primarily operate from home, with the expectation of using virtual communication tools to engage with clients and collaborate with colleagues in a hospice setting. • Conduct comprehensive assessments of clients experiencing significant mental illness or severe emotional disturbances, ensuring that all evaluations are evidence-based and developmentally appropriate.
• Establish trust and rapport with clients from diverse backgrounds to create a therapeutic environment conducive to healing and growth.
• Collaborate closely with other healthcare professionals to create and execute integrated care plans that support the psychological and emotional needs of clients in hospice care.
• Facilitate communication between clients, family members, and healthcare providers to address and resolve concerns effectively.
• Provide short-term intervention and therapeutic services that are tailored to individual client needs, focusing on empowerment and self-determination.
• Monitor client progress, adjusting treatment plans as necessary to ensure optimal outcomes and adherence to ethical standards.
• Participate actively in team meetings and committed professional development opportunities to enhance skills and collaborate on shared goals.

This position primarily serves clients within hospice care settings who exhibit a variety of psychological and emotional challenges. The ideal candidate will be adept at navigating the complexities of end-of-life issues and providing compassionate support to clients and their families. A deep understanding and sensitivity to the cultural and individual diversities within this population are crucial for delivering effective, respectful, and personalized care.

• Master's Degree in Social Work (MSW) or a closely related field is required.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Illinois.
• Minimum of two years of clinical experience is preferred, with a focus on mental health treatment.
• Expertise in assessment, diagnosis, crisis management, and treatment planning for mental health conditions.
• Strong verbal and written communication skills, enabling effective interaction with clients, families, and team members.
• Proven organizational and planning abilities in managing caseloads and treatment schedules.
• Competency in addressing the age-related or specialty issues relevant to the client demographics.

• Bilingual proficiency in English and Spanish is highly desired and advantageous for better service delivery.
• Experience within a hospice or palliative care setting is preferred.
