We are seeking a compassionate and dedicatedBachelor of Social Work (BSW) or Master of Social Work(MSW) professional to join our dynamic care team!
Position Overview As aTeleMate Health Social Worker, you will play a vital role in supporting patients' overall health and wellness through resource coordination, education, and advocacy. Working closely with the care team, you'll help patients overcome barriers to care, promote engagement, and improve quality of life. This position offers work-life balance, and the opportunity to make a meaningful impact on patients' lives from the comfort of your home.
Key Responsibilities

  • Conduct basic psychosocial assessments to identify patients' needs, challenges, and available supports.
  • Collaborate with the care team to create individualized plans addressing social and environmental factors that impact health.
  • Connect patients with appropriate community resources, benefit programs, and support services.
  • Provide education to patients and caregivers on navigating healthcare systems and available resources.
  • Serve as a patient advocate, ensuring care aligns with the patient's goals and preferences.
  • Communicate with healthcare providers and agencies to promote coordination of care.
  • Accurately document all interactions and interventions in the electronic health record (EHR).
  • Monitor patient progress and follow up to ensure needs are met.
  • Identify and escalate urgent psychosocial or behavioral health concerns to appropriate team members.


Qualifications and Skills

  • Bachelor's degree in Social Work (BSW) from an accredited program.
  • Current, active social work licensure
  • Strong understanding of community resources and social determinants of health.
  • Excellent communication and organizational skills.
  • Compassionate, patient-centered approach with strong advocacy skills.
  • Ability to work independently and manage time effectively in a remote environment.
