Serene Reflections Therapy is seeking a dedicated Senior Social Worker to join our compassionate team, serving clients in Fort Lauderdale, Florida. The ideal candidate will hold a Master's degree in Social Work and possess current licensure as a Licensed Master Social Worker (LMSW) in the state of Florida.

Our Social Workers engage in regular communication with an interdisciplinary team of professionals, including nurses, caregivers, and health specialists, to ensure seamless service delivery. This role encompasses a variety of critical tasks aimed at supporting our senior clients:

• Conducting comprehensive assessments of seniors’ psychosocial needs to understand emotional, mental, and social factors influencing their well-being.
• Co-creating tailored care plans with clients and their families, focusing on specific goals and preferences to maximize their quality of life.
• Providing individual counseling and emotional support to clients, guiding them through transitions such as loss, health changes, and adjusting to new living situations.
• Advocating for clients’ rights and connecting them to beneficial community resources and services.
• Collaborating with an interdisciplinary team to ensure holistic care and improve client outcomes.
• Performing regular client visits to evaluate living conditions, address concerns, and monitor health changes.
• Facilitating communication and conflict resolution among family caregivers to foster positive relationships and ensure comprehensive support.
• Educating clients and families about community resources and support networks to help them stay engaged and informed.
• Maintaining accurate and thorough documentation of client interactions, assessments, and progress notes in compliance with regulatory standards.
• Staying updated on current best practices and policies in social work, particularly relating to geriatric care.

This position primarily focuses on older adults who are experiencing various socio-emotional and psychological challenges. These may include dealing with health-related changes, loss of loved ones, and difficulties in maintaining their independence. Your role will significantly impact their lives by providing guidance and support, enhancing their ability to cope with such transitions and promoting overall well-being.

• Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
• Minimum two years of professional experience in a social work role, preferably within a community health or geriatric setting.
• In-depth understanding of social work principles, emotional aging processes, and available community resources that can assist seniors.
• Excellent assessment, counseling, and problem-solving skills.
• Ability to empathize, demonstrate compassion, and build rapport with seniors and their families.
• Strong communication and interpersonal skills necessary for teamwork and collaboration.
• Proficiency in using electronic health records (EHR) and other relevant computer applications.

Experience with geriatric clients and knowledge of mental health issues related to aging can enhance your effectiveness in this role. Additionally, familiarity with community resources available to seniors within the Fort Lauderdale area will aid in more effective advocacy and support.
