Work Environment

Key Responsibilities

• Conduct thorough evaluations of clients' psychosocial needs to create individualized care plans addressing emotional, mental, and social factors.
• Collaborate with clients and their families to establish clear goals and develop tailored intervention strategies.
• Provide empathetic counseling and emotional support to guide seniors and their families through transitions and challenges.
• Advocate for clients' rights and facilitate access to essential community resources, ensuring they receive comprehensive support.
• Work within an interdisciplinary team that includes healthcare professionals, caregivers, and family members to coordinate effective care strategies.
• Regularly visit clients to monitor their well-being, maintain a safe living environment, and evaluate any changing needs.
• Educate seniors and their families on community resources, support networks, and social activities to promote engagement and quality of life.
• Document and maintain accurate records of client assessments, care plans, and interactions in compliance with ethical standards.
• Stay current on best practices in social work and changes in relevant policies and regulations to continuously improve service delivery.

Qualifications

• A Master's degree in Social Work from a recognized institution.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
• A minimum of two years of experience as a social worker, ideally in a community center, healthcare, or geriatric environment.
• Proficiency in using electronic health records (EHR) systems and relevant computer applications.
• Demonstrated understanding of social work principles and the specific needs of the aging population.
• Ability to communicate with compassion, empathy, and professionalism, establishing trust with clients and families.
• Strong analytical, problem-solving, and organizational skills necessary for effectively managing caseloads.

Preferred Experience

Experience working with interdisciplinary teams and familiarity with social assistance programs, geriatric issues, and community resources is preferred. Knowledge of best practices in crisis intervention and elder advocacy will be beneficial.
