Work Environment

This position is rooted in a remote work setting, allowing you the flexibility to connect with clients from the comfort of your home while still providing high-quality support. Key Responsibilities

• Conduct thorough assessments of seniors’ psychosocial needs, incorporating emotional, mental, and social factors to understand each client’s unique circumstances.
• Collaborate with clients and their families to devise personalized care plans that reflect their specific goals, preferences, and cultural considerations.
• Provide emotional support and counseling to both seniors and their families, facilitating coping strategies to help them navigate various life transitions.
• Act as an advocate for clients, ensuring their rights and access to essential community resources are upheld and supported throughout their care journey.
• Regularly monitor clients’ well-being through scheduled visits, assessing living conditions and addressing any safety or quality-of-life issues that arise.
• Foster effective communication among family members involved in the seniors' care, addressing conflicts and encouraging collaborative problem-solving.
• Educate clients and their families on available support systems, resources, and community programs designed to facilitate social engagement and enhance quality of life.
• Maintain accurate and up-to-date documentation of client interactions, assessments, care plans, and progress notes, ensuring compliance with regulatory standards.
• Stay informed of current best practices, policies, and ethical guidelines in social work to provide the best possible care.

Qualifications

• Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

Compensation and Benefits

Serene Reflections Therapy offers a competitive salary range of $60,000 to $100,000 annually, commensurate with experience and qualifications.
