Bright Path Social Work is seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to join our dedicated team. This position is fully remote, allowing you the flexibility to manage your schedule while ensuring seniors receive the support they need. Our Detox Center provides an essential service for maintaining the health and safety of clients undergoing rehabilitation. While the core work hours are from Monday to Friday, 8:00 AM to 4:30 PM, occasional evening or weekend consultations may be required based on client needs. You may also be on-call, addressing urgent situations as they arise. We prioritize a collaborative work culture that encourages communication and teamwork among all staff members.

• Conduct thorough psychosocial assessments to evaluate the needs and circumstances of seniors, identifying emotional, mental, and social challenges they may face.
• Develop, implement, and review personalized care plans, working closely with clients and their families to reflect their unique goals and preferences for care.
• Provide individual and family counseling to help clients navigate the emotional intricacies of aging, transitions, and life changes.
• Advocate for clients to ensure their rights are upheld and that they have access to essential community resources and services.
• Collaborate with an interdisciplinary team of healthcare professionals, including nurses, caregivers, and physicians, to coordinate comprehensive care plans that address the holistic needs of each client.
• Conduct regular home visits and virtual check-ins to monitor clients’ progress, assess their living conditions, and address any emerging concerns.
• Facilitate communication within families to resolve conflicts or misunderstandings concerning client care.
• Educate clients and their families about community resources, support groups, and programs designed to enhance social engagement and support mental health.
• Maintain accurate and detailed records of assessments, progress notes, and care plans, ensuring compliance with ethical standards and regulations.
• Stay updated on current social work practices, policies, and trends in geriatric care, ensuring the highest quality service delivery.

Your primary focus will be on seniors, who may be navigating complex health and social challenges. This client population often requires tailored support for issues such as mental health conditions, social isolation, and transitioning to new living arrangements. Understanding the unique needs of older adults is crucial in providing effective interventions and facilitating community integration.

• Master's degree in Social Work from an accredited institution, with a focus on gerontology preferred.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
• At least two years of experience in social work, particularly in a community setting with seniors or in a healthcare environment.
• In-depth knowledge of social work principles, aging-related issues, and local community resources available for seniors.
• Strong assessment, problem-solving, and counseling skills.
• Ability to build rapport and demonstrate empathy with clients and their families.
• Demonstrated communication and interpersonal abilities that facilitate teamwork with various healthcare professionals.
• Familiarity with electronic health record (EHR) systems and proficiency in relevant software applications.

• Previous experience in geriatric social work or related fields will be viewed favorably.
• Experience in crisis intervention and conflict resolution.
• Knowledge of Medicaid and Medicare services as they apply to senior care.
