While working from home, your primary responsibilities will focus on assessing clients' needs, developing care plans, and providing therapeutic support.

• Conduct comprehensive assessments of seniors to evaluate their psychosocial needs, focusing on emotional, mental, and social concerns.
• Collaborate closely with clients and their families to create personalized care plans that align with their goals and preferences.
• Deliver counseling and emotional support, assisting seniors and their families as they navigate significant life transitions and challenges.
• Advocate for clients' rights, ensuring they have access to necessary community resources and services to enhance their well-being.
• Work collaboratively as part of an interdisciplinary team, coordinating with nurses, caregivers, and other healthcare professionals to optimize client care.
• Monitor client well-being through regular assessments, adjusting care plans as required to address any emerging concerns.
• Facilitate effective communication among family members involved in the care processes and help resolve conflicts as they arise.
• Educate clients and families regarding available community resources, support groups, and healthcare programs to foster their social engagement.
• Maintain up-to-date documentation, including assessments, care plans, and progress notes, ensuring compliance with ethical standards and organizational policies.
• Stay informed of current social work practices, policies, and regulations to uphold a high standard of care.

• Prior experience working in a remote setting and familiarity with telehealth services is highly desirable.
• Experience in organizing and leading support groups for seniors.
• Knowledge of local community resources and senior service organizations within Coral Springs, Florida.
