This position is set within a remote environment, allowing for flexibility in how support is delivered to our clients at the Detox Center. Despite the remote nature, collaboration remains a cornerstone of our practice; you will work closely with interdisciplinary teams including nurses, caregivers, and other healthcare professionals to ensure the most effective care is provided.

• Carry out thorough psychosocial assessments for the elderly, factoring in emotional, mental, and social determinants relevant to their well-being.
• Collaborate with clients and their families to craft personalized care plans tailored to meet individual goals and preferences.
• Provide counseling and emotional support to clients, guiding them through life changes and minimizing challenges.
• Act as an advocate for clients' rights, ensuring they have seamless access to community resources and necessary support services.
• Engage with interdisciplinary teams to facilitate comprehensive care, participating in case discussions that focus on the health and well-being of clients.
• Conduct regular virtual visits to assess client conditions and ensure their living environments are safe and supportive.
• Serve as a mediator and resource for families, helping to resolve conflicts and improve communication among all involved in the client’s care.
• Educate clients and families about available resources, support groups, and programs to promote social engagement and enhance quality of life.
• Maintain precise and timely documentation of assessments, care plans, progress notes, and significant interactions within our electronic health record system.
• Stay aligned with current trends, practices, and regulations in social work, maintaining compliance with all ethical standards.

Your primary focus will be on providing support to seniors, particularly those grappling with issues related to aging, mental health, end-of-life transitions, and familial challenges. Understanding the unique needs of this population is essential in guiding your interactions and interventions.

• Master’s degree in Social Work from an accredited program is required.
• Active and valid state licensure as an Independent Clinical Social Worker (LICSW).
• A minimum of two years of professional experience in social work, ideally in settings serving seniors or communities.
• In-depth knowledge of social work principles, as well as insights into aging-related issues and available community resources.
• Exceptional assessment, counseling, and problem-solving capabilities.
• A strong capacity for empathy and communication skills, capable of building rapport with clients and their families.
• Ability to effectively collaborate with interdisciplinary teams.
• Familiarity with electronic health records (EHR) systems and general computer applications is crucial.

• Experience in a community center or geriatric social work environment is highly advantageous.
