Key Responsibilities
  • Carry out thorough psychosocial assessments for the elderly, factoring in emotional, mental, and social determinants relevant to their well-being.
  • Collaborate with clients and their families to craft personalized care plans tailored to meet individual goals and preferences.
  • Provide counseling and emotional support to clients, guiding them through life changes and minimizing challenges.
