Trinity Health Of New England is looking for a Licensed Social Worker to join our Outpatient Heart Failure team.
The Social Worker LCSW will support our Outpatient Heart Failure team located within Saint Francis Hospital within our Hospital Outpatient Department (HOD). The LCSW will assess and assists in addressing and meeting the psycho-social needs of heart failure patients and families, enabling patients to utilize medical care and other supportive services to achieve their optimum level of health and function.

You Will Be Responsible For:

  • Assesses and counsels' patients/families referred to the Outpatient Heart Failure Program
  • Provides consultation to physicians and staff in ambulatory planning, from treatment, to psychosocial distress assessment
  • Patient centered counseling and consultative responsibilities required based on individual needs assessment
  • Communicates via written documentation in the clinical record and through verbal discussions, all pertinent information on patient status to ensure that the team understands patient's circumstances relative to achieving the plan of care

Requirements:

  • Education: master's degree required
  • Valid Social Worker License from the state of CT, Licensed Master of Social Work (LCSW) required
  • Two (2) years of related professional experience required
  • Ability to communicate effectively both orally and in writing. This may include explaining clinical, technical, and diagnostic procedures to clients and their families, counseling clients and their families and enforcing policies and regulations in difficult situations
  • Work requires the analytical ability to collect information from diverse sources, apply professional principles in performing various analyses, and summarize the information and data to solve problems or design complex systems and programs
