• Conduct thorough clinical assessments of patients to identify psychosocial needs, emotional states, and support requirements.
• Develop and implement individualized treatment plans that incorporate evidence-based therapeutic interventions tailored to the unique circumstances of each patient and their families.
• Maintain accurate and timely clinical documentation to comply with organizational and regulatory standards.
• Collaborate effectively with an interdisciplinary team consisting of physicians, nurses, chaplains, and other healthcare professionals to provide holistic patient care.
• Provide education and emotional support to patients and family members about the psychosocial aspects of end-of-life issues.
• Participate in regular team meetings and case conferences to discuss patient care strategies and outcomes while contributing valuable insights from a social work perspective.
• Stay abreast of current mental health trends, research, and best practices to enhance the quality of care provided to our patients.

This role will involve working closely with patients at various stages of life-limiting illnesses and their families. You will provide critical support during sensitive times, helping individuals and families adjust to and cope with their circumstances, and offering grief support as needed. Your efforts will directly impact their emotional well-being and overall experience during hospice care.

• A Master’s degree in Social Work from an accredited institution.
• A valid Licensed Clinical Social Worker (LCSW) license in the state of Florida.
• In-depth understanding of solution-focused therapy and psychotherapeutic interventions tailored to hospice care settings.
• Strong communication, problem-solving, and empathy skills, with the ability to navigate sensitive topics with compassion and professionalism.

Candidates with at least two years of experience in clinical social work, particularly within a healthcare or hospice environment, will be highly desirable. Experience in crisis intervention, grief counseling, or working with diverse populations is a plus, as it will enhance your ability to effectively support our patients and their families.
