Work Environment

This position is fully remote, offering you the flexibility to work from your chosen environment while still being an integral part of our collaborative team. The work setting involves frequent communication with clients over virtual platforms to facilitate assessments and deliver counseling. Although the primary role is remote, engagement with interdisciplinary teamsโ€”including healthcare professionals, caregivers, and familiesโ€”is essential to ensure comprehensive care coordination. Our regular work schedule is from Monday to Friday, 8:00 AM to 4:30 PM, with some flexibility required for evening or weekend appointments.

Key Responsibilities

  • Develop and implement personalized care plans alongside clients and their families, focusing on actionable steps to achieve specific goals.
  • Provide ongoing counseling sessions to support seniors facing significant life transitions or mental health challenges.
  • Advocate on behalf of clients to help them access appropriate community resources, ensuring they receive crucial support services.
  • Collaborate closely with interdisciplinary teamsโ€”such as nurses and healthcare professionalsโ€”to enhance client support and monitor overall well-being.
