Work Environment

This position is primarily remote, allowing for flexibility while maintaining a strong commitment to client care. The work setting includes collaboration with an interdisciplinary team of healthcare providers, ensuring a coordinated approach to care that prioritizes the complex needs of our clients. While you will mainly work from home, occasional visits to our Hospice Center may be necessary to assess client needs in person or attend team meetings.

Key Responsibilities

• Conduct comprehensive assessments of seniors' psychosocial needs, evaluating emotional, social, and mental factors affecting their well-being.
• Collaborate closely with clients and their families to develop individualized care plans that reflect their personal goals and resources.
• Provide counseling and emotional support, helping clients navigate significant transitions such as aging, loss, and caregiving challenges.
• Advocate for clients’ rights and access to essential community resources, ensuring they receive appropriate support and services.
• Work as part of an interdisciplinary team with nurses, caregivers, and other healthcare professionals, coordinating care to enhance overall client outcomes.
• Monitor clients' well-being through regular check-ins, assessing living conditions and addressing any safety concerns that may arise.
• Facilitate effective communication among family members involved in the care of seniors, helping to resolve conflicts and promote collaborative care.
• Educate clients and families about available community resources, support groups, and wellness programs to improve their quality of life.
• Provide meticulous documentation related to client assessments, care plans, progress notes, and compliance with industry regulations.
• Stay informed about contemporary social work practices, policies, and ethical standards to ensure compliance and high-quality service delivery.

Patient Population / Client Focus

The ideal candidate will be dedicated to serving the senior population, along with their families. You will engage with clients dealing with various challenges, including mental health conditions, grief, social isolation, and the complexities of aging. Your work will help empower them to regain control over their emotional well-being and enhance their overall quality of life.

Qualifications

• A Master's degree in Social Work from an accredited institution is required.
• Current and valid licensure as a Clinical Social Worker (LCSW) in Kansas.
• A minimum of two years of professional experience in social work, ideally in a community or geriatric setting.
• In-depth knowledge of social work principles, aging-related issues, and local community resources.
• Strong assessment, counseling, and problem-solving abilities that demonstrate empathy and compassion.
• Exceptional communication and interpersonal skills for effective collaboration within a multidisciplinary team.
• Proficiency in utilizing electronic health record (EHR) systems and other relevant technology to maintain documentation.

Preferred Experience

• Experience working specifically with senior clients and their families.
• Knowledge and understanding of end-of-life care and hospice services.
• Demonstrated capability in crisis intervention and managing complex family dynamics.

Compensation and Benefits
