• Conduct comprehensive mental health assessments, considering the full spectrum of patient conditions—including physical health issues that may impact mental wellbeing.
• Develop and implement evidence-based integrated treatment plans tailored to the unique needs of each patient, addressing both psychiatric and medical co-morbidities.
• Facilitate individual and group therapy sessions, employing a variety of therapeutic modalities to support patient growth and resilience.
• Work collaboratively with medical providers, sharing insights and recommendations to fine-tune treatment plans and improve patient outcomes.
• Provide timely crisis intervention during mental health emergencies, utilizing best practices to ensure patient safety and well-being.
• Educate patients about the connections between their mental health and physical conditions, equipping them with skills to manage their wellbeing proactively.
• Maintain meticulous and confidential patient records, ensuring compliance with legal and ethical guidelines.
• Engage actively in team discussions, case conferences, and interdisciplinary care planning to promote a unified treatment approach.
• Stay informed about advancements in integrated mental health care practices and participate in professional development opportunities.

Our diverse patient population includes individuals dealing with a variety of complex conditions such as autoimmune disorders, chronic pain, and substance use disorders. Many of our clients seek support for managing dual diagnoses, and your role will be crucial in helping them address the interplay of their mental health and physical health challenges.

• Master's or Doctoral degree in a relevant field such as Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Current and valid licensure as an LCSW, LMHC, or LMFT in New York State.
• Demonstrated ability to address mental health needs in individuals with comorbid medical conditions.
• Outstanding interpersonal and communication skills, with a talent for building rapport with clients.
• Proficiency with electronic health record systems and telehealth tools to streamline service delivery.
• A commitment to maintaining high professional standards, ethical guidelines, and patient confidentiality.

• Experience working within an integrated care framework is highly desirable.
• Experience in providing therapy through telehealth platforms.
• Knowledge of trauma-informed care principles.

The position offers a competitive salary range of USD 90,000 – 100,000 annually, commensurate with experience.
