• Conduct thorough assessments of seniors' psychosocial needs, incorporating emotional, mental, and social considerations.
• Collaborate closely with clients and their families to develop individualized care plans that reflect their preferences and goals.
• Provide counseling and emotional support to seniors navigating life transitions, health challenges, and end-of-life issues.
• Act as an advocate for clients' rights by ensuring access to necessary community resources and services.
• Engage with an interdisciplinary team of healthcare professionals—including nurses and caregivers—to enhance the care provided to clients.
• Monitor client well-being through regular virtual visits, evaluating living conditions and addressing safety concerns.
• Facilitate effective communication among family members involved in care, addressing any conflicts that may arise.
• Educate clients and families about available community resources and support groups to improve social engagement.
• Maintain detailed and accurate documentation of assessments, care plans, progress notes, and client interactions.
• Continuously update your knowledge of social work practices and comply with all relevant regulations and ethical standards.

This role focuses primarily on seniors within a school-based environment, where you will work collaboratively to support both their mental health and community engagement. • A Master’s degree in Social Work from an accredited institution is required.
• A current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

• Strong knowledge of social work principles, aging-related issues, and community resource availability.
• Exceptional assessment, counseling, and problem-solving abilities.
• Demonstrated capacity to empathize and connect with seniors and their families.

• Proficiency in using electronic health records (EHR) and various computer applications to manage client information.

• Experience working in educational or school settings with senior clients.
