Job Description Summary
The Population Care Nurse is a registered nurse (RN) who focuses on managing and improving the health outcomes, wellness and quality performance of specific populations or communications, particularly those with chronic conditions, high healthcare needs, or underserved groups. This role involves working within interdisciplinary teams to assess, plan and deliver patient care at a population level, rather than focusing on individual acute care.
Job Description
The Population Care Nurse implements population intervention to improve health, wellness and quality performance. The interventions include evidence-based preventative services such as immunization, chronic disease preventative care, cancer screening, medication follow up, and wellness visits. The nurse implements provider-based interventions to improve health, wellness and quality performance. Additionally, the nurse is involved in preventative care, health education, case management, and coordination of services to ensure individuals within the population receive comprehensive care, with a focus on reducing hospital readmissions, managing chronic illnesses and addressing health disparities.
Education:
  • Bachelors Degree in Nursing REQUIRED

Certification/Licensure
  • Registered Nurse (RN) License (Compact or Virginia) REQUIRED

Experience:
  • 2 years Nursing experience REQUIRED
  • Health Plan experience preferred
