About the job

Vynca is seeking a dedicated Registered Nurse (RN) to play a pivotal role in our Clinical Care Delivery Team! As a RN with Vynca, you are a key part of the interdisciplinary team, helping to improve care through evidence-based guidelines for seriously ill patients through a hybrid model providing in-person visits and telemedicine visits. Weโ€™re seeking someone with a genuine passion for this important work and a deep commitment to providing exceptional patient care. This is a hybrid role that requires traveling throughout northern New Jersey, combining in-person patient visits with telehealth care delivered across NJ and other states where Vynca operates. Candidates wishing to be considered must reside within 25-miles of the assigned territory due to frequency of travel.

What youโ€™ll do

Patient Care

  • Serve as the primary coordinator for your patientโ€™s care needs, facilitating services across the interdisciplinary care team to ensure all patient needs are being met

  • Manage patients assigned to your Care Team, ensuring the RN and Interdisciplinary team visit frequencies align with the patientโ€™s acuity level and clinical needs.

  • Communicate with patients via phone and video conferencing as well as provide in-person visits as needed and as outlined in the care plan.

  • Provide patient care within your scope of practice and per provider orders.

  • Assess and enroll patients, completing initial referral follow-ups as needed.

  • Develop and update care plans based on ongoing assessments and clinical needs as determined by the care team.

  • Collaborate with your care team to ensure patientโ€™s needs are being addressed as their clinical status changes

  • Manage patient medications, including implementing orders, maintaining current medication lists, assisting with medication refills, and assisting patients with setting up consistent medication administration methods.

  • Provide patient follow-up and education on new medication orders.

  • Facilitate requisition of medical supplies and durable medical equipment.

  • Engage in care coordination activities, ensuring timely documentation updates and communication with primary care providers, other community providers, care team members including family and caregivers, home health agency staff, and any other care provider.

  • Coordinate with the Interdisciplinary team and external agencies as necessary, including facilitating advance care planning and hospice referrals.

  • Educate patients, families, and caregivers on Vynca services, anticipatory guidance related to the patientโ€™s serious illness and potential complications, symptom management, and medication trackingโ€”reinforcing the importance of contacting the care team first to help prevent exacerbations and unnecessary hospital admissions.

  • Perform additional duties as assigned.

Float Nurse

  • Assist in covering patient care for other care teams during normal working hours.

Your experience and qualifications
  • Active, unrestricted New Jersey Registered Nurse (RN) license, with the ability and willingness to obtain licensure in other states.

  • 1+ year experience as a professional nurse within the previous three years.

  • Excellent oral and written communication skills required

  • Must have general computer skills and a working knowledge of MS Office and the internet.

  • Willing and able to work Monday through Friday 8:30am to 5pm Eastern Time, both in the field and remotely, with flexibility for potential evenings and weekends.

Preferred Qualifications

  • California and/or Oregon RN license, highly preferred.

  • Hospice or palliative care experience, highly preferred.

  • CHPN certification, or ability to obtain.

  • Remote work experience.
