What You’ll Be Doing
Clinical Triage
 Provide after-hours telephone triage for Hospice, and Provider Services patients
 Assess patient symptoms and determine the appropriate level of care
 Provide evidence-based symptom management recommendations
 Determine when field visits are necessary and coordinate appropriate follow-up
 Collaborate with providers regarding medications, treatment plans, and physician orders
Clinical Support
 Serve as the primary clinical resource for after-hours field nurses
 Support on-call RNs and LPNs with clinical questions and complex patient situations
 Provide real-time guidance regarding symptom management, patient care, and agency processes
 Escalate complex concerns to leadership when appropriate.     
Place orders for after-hours medications refill needs and coordinate with the pharmacy.


Care Coordination
 Coordinate after-hours visits with hospice runners and on-call clinicians
 Process physician orders and scheduling workflows
 Ensure timely communication with each patient’s care team
 Maintain continuity of care across Home Health, Hospice, and Provider Services
Quality & Compliance
 Complete accurate documentation within Homecare Homebase (HCHB)
 Conduct routine chart audits for quality and regulatory compliance
 Participate in QAPI initiatives and performance improvement projects
 Assist with CAHPS and patient satisfaction initiatives
 Identify opportunities for service recovery and quality improvement
Customer Experience
 Complete welcome calls for newly admitted hospice patients and families
 Conduct routine patient and caregiver satisfaction calls
 Build positive relationships with facility partners
 Help ensure every patient and family experiences Riverside’s commitment to Life Changing Service

What You’ll Need
 Active Oregon Registered Nurse (RN) license
 Graduate of an accredited school of nursing
 Current CPR certification
 Minimum of two years of nursing experience
 Hospice experience strongly preferred
Hospice triage experience strongly preferred
Homecare Homebase (HCHB) experience required
 Excellent clinical assessment and critical-thinking skills
 Strong communication and documentation abilities
 Ability to work independently with minimal supervision
 Excellent organization and time-management skills
 Comfortable providing clinical guidance to fellow nurses and collaborating with physicians and interdisciplinary teams
