• Conduct comprehensive assessments of patients’ psychosocial needs, strengths, and challenges to inform individualized care plans.
• Design and implement targeted therapeutic interventions that address emotional, social, and cognitive well-being, promoting optimal recovery paths.
• Facilitate regular updates to treatment plans based on patient progress, ensuring adaptation to their changing needs.
• Maintain accurate, organized, and confidential records in compliance with legal and ethical standards.
• Engage in ongoing collaboration with an interdisciplinary treatment team, including physicians, nurses, and other healthcare professionals, to coordinate comprehensive care.
• Participate in case conferences to share insights, strategies, and progress relative to patient care and rehabilitation.
• Stay informed about the latest evidence-based practices and regulations in social work and hospice care to enhance service delivery.

The ideal candidate will serve a diverse patient population in hospice care, encompassing individuals experiencing complex health issues, trauma, anxiety, and depression. Your work will focus on providing compassionate support to individuals at critical moments in their lives, facilitating access to necessary resources, and ensuring that their emotional and psychological needs are adeptly addressed.

• A Master’s degree in Social Work from an accredited university, demonstrating a solid educational foundation in therapeutic techniques and frameworks.
• Possession of an active Licensed Clinical Social Worker (LCSW) license in Georgia, which allows you to engage legally in clinical practice.
• Strong skills in communication, active listening, and problem-solving, essential for effectively interacting with patients and healthcare teams.
• Ability to thrive in a team-oriented environment while managing the duties of autonomy in a remote work setting.

While not mandatory, prior experience in hospice, palliative care, or rehabilitation settings is highly preferred.
