Key Responsibilities
  • Conduct thorough psychosocial assessments to evaluate patients' mental health status, strengths, and needs related to their illness.
  • Provide evidence-based therapies, including cognitive-behavioral therapy and dialectical behavior therapy, tailored to individual patient circumstances.
  • Collaborate closely with psychiatrists, nurses, and other healthcare professionals on the hospice team to develop coordinated care plans that address the comprehensive needs of patients.
  • Educate patients and their families about diagnoses, treatment options, medications, and coping strategies; empower them to make informed decisions about their care.
  • Facilitate individual, group, and family therapy sessions utilizing modalities supported by research to promote emotional healing and resilience.
  • Develop and regularly update treatment plans in collaboration with the broader treatment team, ensuring patients' goals and preferences are central to their care.
  • Advocate for patients navigating the complex healthcare system, ensuring their rights and needs are prioritized throughout the process.
  • Lead therapeutic groups, psychoeducation sessions, and discharge planning meetings to prepare patients and families for transitions in care.
