This role is designed to be performed remotely, allowing for flexibility and work-life balance while maintaining a strong connection with our community at the Hospice Center. You will collaborate with other healthcare professionals and engage in regular team meetings, utilizing telehealth platforms and integrated electronic health record systems to communicate effectively and manage client information securely.

• Conduct thorough assessments of clients' mental health and emotional well-being, identifying their needs and developing comprehensive treatment plans.
• Implement behavior management strategies tailored to individual client situations, ensuring a supportive and therapeutic environment at all times.
• Provide both individual and group counseling sessions, utilizing therapeutic techniques to address grief, anxiety, and depression commonly experienced in hospice care.
• Work alongside a multidisciplinary team that includes physicians, nurses, and chaplains to create a holistic support system for clients and their families.
• Educate and empower family members to facilitate growth and coping during challenging times, adhering to best practices in family therapy.
• Utilize evidence-based psychotherapy methodologies to enhance clients' coping mechanisms and mental resilience.
• Maintain accurate and timely clinical documentation in accordance with all applicable regulations and standards, using electronic health record systems effectively.

In this role, you will work primarily with clients and families facing terminal diagnoses, providing not only emotional support but also valuable resources to navigate end-of-life decisions. This patient population is diverse, and cultural competence is essential in recognizing and addressing the unique values and beliefs of each client.

• A Master’s degree in Social Work from an accredited institution is required.
• A valid and active LCSW license in California is essential for this position.
• Minimum of two years of clinical experience, specifically within hospice or palliative care settings, is necessary.
• Proficiency in utilizing electronic health record systems and telehealth technology is expected.
• Strong analytical, organizational, and communication skills are critical for effective client interactions and documentation.

• Experience working with a diverse range of clients, particularly those dealing with grief or related issues.
• Familiarity with interdisciplinary team dynamics in healthcare settings will be beneficial.
• Training in evidence-based therapies, such as Cognitive Behavioral Therapy (CBT) or Acceptance and Commitment Therapy (ACT), is a plus.

The salary range for this position is competitive, offering between $110,000 and $130,000 annually, commensurate with experience. Benefits include:
