Love Bound Therapy is actively seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our team in providing crucial support for seniors in a hospice center setting. This full-time remote role is designed for an experienced social worker who is committed to enhancing the quality of life for seniors and their families through empathetic care and professional guidance.

Work Environment

This position operates remotely, allowing for flexible engagement with clients while upholding the high standards of care our hospice center is known for. The work schedule typically runs Monday through Friday from 8:00 AM to 4:30 PM, although occasional evening or weekend hours may be necessary to accommodate client needs. Moreover, on-call shifts may be required to address urgent situations or provide support during off-hours, demanding both adaptability and responsiveness in a dynamic care environment.

Key Responsibilities
  • Conduct comprehensive assessments of clients to identify their psychosocial needs, integrating emotional, mental, and social aspects into care planning.
  • Collaborate with clients and their families to design tailored care plans that reflect individual goals, preferences, and cultural values.
  • Provide counseling and emotional support, helping seniors and their families navigate transitions such as illness or loss with dignity and respect.
