Self Harmony Counseling is seeking a dedicated and licensed Master Social Worker (LMSW) to join our compassionate team, focusing on the well-being of seniors in Erie, Pennsylvania. This full-time remote position plays a crucial role in enhancing the quality of life for our senior clients by providing vital social work services that address their emotional, mental, and social needs.

This role is designed to be conducted remotely, offering flexibility while still requiring some on-site services at a detox center when needed. Our working hours are primarily Monday through Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend hours based on client needs. Flexibility is essential as you accommodate visits and ensure consistent, high-quality care.

As a Licensed Master Social Worker, you will:

• Conduct comprehensive assessments of the psychosocial needs of seniors, examining emotional, social, and mental health aspects.
• Collaborate closely with clients and their families to develop personalized treatment plans that reflect their specific needs and objectives.
• Provide counseling and emotional support, empowering seniors to navigate challenges such as bereavement, health changes, and family dynamics.
• Advocate for clients' rights, ensuring access to critical healthcare services and community resources that enhance their quality of life.
• Work alongside a diverse interdisciplinary team of healthcare professionals, including nurses and caregivers, to deliver coordinated and effective care.
• Perform regular visits to evaluate clients’ living conditions, monitor their mental health, and address any evolving concerns.
• Facilitate family communications, helping to mediate conflicts and promote understanding among family members involved in a client's care.
• Educate clients and their families about valuable community resources, including support groups and programs aimed at improving social engagement.
• Maintain meticulous documentation of client interactions, assessments, care plans, and progress, ensuring compliance with legal and ethical standards.
• Stay updated on best practices, policies, and operations within the social work field, contributing to the continuous improvement of our service delivery.

The primary focus of this position is on senior citizens, many of whom may be living in assisted living facilities or receiving care in a detox center. These clients often experience complex emotional and psychosocial challenges, including depression, anxiety, cognitive decline, and the impacts of chronic illness. Your role is pivotal in not just addressing their immediate needs but also in helping them and their families embrace and adapt to the aging process.

• A Master’s degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
• A minimum of two years of experience in a social work role, preferably within a community center or geriatric care setting.
• Strong knowledge of social work principles, aging-related issues, and available resources in the Erie community.
• Excellent assessment, counseling, and problem-solving capabilities.
• Empathy, compassion, and the ability to establish a meaningful rapport with senior individuals and their families.
• Outstanding communication and interpersonal skills for effective collaboration.
• Proficient in electronic health record (EHR) systems and associated software applications.

• Experience working with senior populations in varied settings such as residential facilities, detox centers, or community health organizations.
• Proven background in conducting workshops or support groups tailored to seniors and their caregivers.
• Knowledge of local community resources, support services, and advocacy strategies relevant to the aging population.
