What is a Remote RN Discharge Planning nurse?

A Remote RN Discharge Planner is a registered nurse who helps coordinate and manage patient discharges from hospitals or healthcare facilities, working remotely rather than onsite. Their responsibilities include assessing patient needs, developing discharge plans, collaborating with healthcare teams, arranging follow-up care, and ensuring a smooth transition from hospital to home or another care setting. By working remotely, these professionals use phone, email, and electronic health records to communicate with patients and care providers, aiming to improve patient outcomes and reduce readmissions.

What are the key skills and qualifications needed to thrive as a Remote RN Discharge Planning nurse?

To thrive as a Remote RN Discharge Planning nurse, you need a solid nursing background with RN licensure, strong clinical assessment skills, and experience in discharge planning or care coordination. Familiarity with case management software, electronic health records (EHRs), and telehealth platforms is typically required. Excellent communication, problem-solving, and organizational skills are crucial for collaborating with patients, families, and interdisciplinary teams remotely. These capabilities ensure safe, effective transitions of care and help reduce readmissions while maintaining high-quality patient outcomes.

Position Overview

The Divine Psychological Wellness Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team on a full-time basis. This position represents an exceptional opportunity for a social worker who is passionate about improving the lives of individuals undergoing detoxification and recovery. Working mainly in a remote setting while serving clients at our specialized detox center, you will play a vital role in coordinating and implementing patient-centered care strategies, offering emotional support, and facilitating discharge planning for our clients.

About the Organization

Divine Psychological Wellness Center is committed to providing holistic and integrative mental health services. Our mission is to empower individuals on their journey to recovery by addressing not only their psychological needs but also the environmental, social, and cultural factors that may impact their health. Nestled in the heart of Santa Rosa, California, we believe in fostering a supportive environment for both our clients and staff, promoting sustained recovery and overall well-being. Your contributions will directly support our mission of transforming lives through compassionate, therapeutic care.

Work Environment

This role offers the flexibility of a remote work environment, allowing you to engage with clients while maintaining a balanced professional life. As part of the detox center team, you will collaborate closely with healthcare professionals, including physicians and nurses, utilizing our advanced electronic health record (EHR) system for documentation and case management. This digital platform enables seamless communication and effective care coordination, ensuring that all team members are aligned in providing the highest quality of support to our clients.

Key Responsibilities
  • Manage a diverse caseload of clients seeking detox services, providing tailored clinical social work services that meet their unique needs.
  • Conduct comprehensive assessments of clients and their families to identify social, emotional, and environmental factors contributing to their challenges.
  • Facilitate care coordination by collaborating with multidisciplinary teams, including psychiatrists, medical staff, and case managers to develop and implement holistic treatment plans.
  • Assist in the discharge planning process by evaluating available resources, making referrals to community services, and providing ongoing support to ensure successful transitions.
  • Document patient interactions accurately in our EHR system, maintaining compliance with healthcare regulations and ensuring accountability in all professional practices.
  • Participate in regular team meetings to discuss cases and share insights that can enhance patient outcomes.
  • Stay updated on best practices in social work, particularly within the context of addiction and recovery.
Patient Population / Client Focus

In this role, you will work primarily with adults who are navigating the complexities of substance use disorders and the detoxification process. You will be equipped to provide emotional support for clients experiencing acute withdrawal symptoms and address the accompanying psychological challenges. Your expertise in age-specific and behavior-specific social work practice will enable you to effectively assist a diverse client population, including those with co-occurring mental health issues.

Qualifications
  • A Master’s degree in Social Work (MSW) from an accredited institution is required.
  • Current license as a Licensed Clinical Social Worker (LCSW) in California.
  • Minimum of 2 years of experience in social work, preferably in an inpatient hospital or community healthcare setting.
  • Demonstrated knowledge and competence in addressing behavioral health issues and applying therapeutic methodologies relevant to addiction recovery.
  • Strong communication and interpersonal skills that foster effective relationships with patients, families, and interdisciplinary team members.
  • Commitment to ethical practice and support of the Patient Bill of Rights.
Preferred Experience
  • Experience working in a detox or rehabilitation setting is highly desirable.
  • Familiarity with assessment and treatment methodologies specific to addiction medicine.
  • Ability to engage with diverse populations and adapt interventions based on individual cultural contexts.
Compensation and Benefits

The annual salary for this position ranges from USD 115,000 to 130,000, commensurate with experience and qualifications. In addition to competitive pay, we offer a robust benefits package that includes:

  • 401(k) retirement plan with company matching contributions.
  • Diverse health insurance options, including medical, dental, and vision coverage.
  • Disability insurance to support you and your family in unexpected circumstances.
  • Employee assistance programs that offer mental health support and counseling for personal and professional challenges.
Career Growth Opportunities

Joining Divine Psychological Wellness Center offers you not only a fulfilling role but also the potential for career advancement within our organization. Opportunities for continued professional development, including workshops, training sessions, and certifications in specialized areas of social work and addiction counseling, will be encouraged and supported. As we grow, so will your career.
