The CalAIM Case Coordinator H supports the successful reintegration of CalAIM-eligible individuals by helping coordinate the development and execution of reentry care plans under the direction of the RN Discharge Planner. This role assists with Medi-Cal eligibility screenings, identifies individuals who qualify for CalAIM Justice-Involved services, and helps coordinate communication and service delivery with managed care plans, providers, and community partners. The position supports continuity of care by facilitating reentry planning activities and ensuring key services and information are coordinated prior to release.

  • Coordinate with Medi-Cal eligibility staff, in-reach providers, and community partners to support CalAIM Justice-Involved service activation and reentry planning.
  • Gather clinical and logistical information, track release dates, obtain required consents, and support warm handoffs for continuity of care.
  • Ensure timely documentation in the EHR and monitor release timelines to support service delivery and compliance requirements.
  • Activate Justice-Involved (JI) aid codes in the state reentry portal and ensure alignment with required pre-release timelines.
  • Support continuity of care and CalAIM compliance under the clinical oversight of the RN Discharge Planner.

30 hours onsite. 10 hours remote

Education

  • Bachelor's degree in Social Work, Business Administration, or other health-related fields from an accredited college or university is preferred

Experience

  • Previous experience in developing successful transition plans involving community resources for persons with medical, substance use, and/or mental illness is preferred.
  • Strong communication, documentation, and coordination skills, as well as a working knowledge of Medi-Cal processes and CalAIM policy requirements, are essential.

Licenses/Certifications

  • Must maintain CPR certification, and any other certifications (such as First Aid) required by contract.
