• Conduct thorough clinical assessments to identify the needs, strengths, and challenges of clients.
• Develop and implement individualized treatment plans that include evidence-based strategies and interventions.
• Utilize solution-focused therapy techniques to facilitate positive change and promote mental well-being.
• Maintain accurate and confidential client records in compliance with regulatory standards and organizational policies.
• Collaborate closely with a team of professionals including nurses, physicians, and other social workers to ensure comprehensive care delivery.
• Participate in regular team meetings to share insights, review client progress, and adjust treatment plans as necessary.
• Stay informed on current research and developments in social work practices, trauma-informed care, and mental health strategies to continually refine your approach.

Your primary focus will be on working with clients in a hospice setting who are experiencing a range of mental health issues, including but not limited to anxiety, depression, grief, and trauma. Many clients may be navigating significant life changes, making it essential to approach care with sensitivity and empathy. You will provide support not just to the patients but also to their families, ensuring that they have the tools and resources they need during difficult times.

• A Master’s degree in Social Work from an accredited institution.
• An active LCSW license in the state of Illinois.
• Strong communication and interpersonal skills, with the ability to establish rapport with diverse populations.
• Proficient in using electronic health records (EHR) systems for maintaining client documentation.
• A commitment to continued professional development and staying current with best practices in the field.

• Experience providing therapy in a hospice or similar healthcare setting.
• Background in trauma-informed care approaches.
• Knowledge of community resources and referral networks for client support.

This position offers a competitive salary ranging from USD 80,000 to 100,000 annually, with the potential for additional bonuses based on performance.
