• Conduct comprehensive assessments for new clients to determine their individual needs and develop tailored treatment plans.
• Maintain clear and ongoing communication with supervisors, support staff, and family members to effectively monitor and report on client progress.
• Create and manage an organized schedule detailing appointments, follow-ups, and home visits as necessary for effective treatment.
• Coordinate and conduct home visits as required, ensuring thorough observational assessments, chart reviews, and staff interviews.
• Implement and monitor therapeutic programs, ensuring alignment with established best practices and organizational standards.
• Actively participate in interdisciplinary team meetings to discuss client care strategies and treatment plan adjustments.
• Prepare and submit monthly and quarterly progress notes that accurately reflect client advancements and challenges.
• Organize workshops and training sessions for staff to enhance their competencies in program execution and documentation practices.
• Facilitate follow-up sessions via interviews and home visits to address ongoing client needs and support their journey toward wellness.
• Act as a liaison between clients, families, and community resources to enhance the delivery of care and support developmental goals.

This role engages with a diverse patient population, including both adults and youth, who present a range of mental health challenges. You will provide culturally sensitive and individualized counseling to clients requiring support in areas such as trauma, substance misuse, family dynamics, and personal development. Your work will be pivotal in promoting mental health awareness and demonstrating the effectiveness of integrated care solutions.

• A minimum of a Bachelor's Degree in Counseling, Psychology, or a related field is required. A Master's or Doctoral degree is preferred.
• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in the state of Illinois.
• At least two years of direct counseling experience, particularly within clinical settings.
• Expertise in specialized areas such as family therapy, couples therapy, trauma recovery, or substance misuse is highly desirable.
• In-depth understanding of ethical and legal standards in counseling practices.
• Demonstrated ability to establish strong rapport with clients, showcasing excellent interpersonal and communication skills.
• Experience collaborating effectively within a multifaceted healthcare team.
• Proficient in using EMR software and related digital tools for documentation and client management.

• Prior work in an outpatient mental health setting, with experience delivering telehealth services being a plus.
• Familiarity with evidence-based treatments and programs to help address a variety of mental health issues.
• Training or certification in trauma-informed care or similar methodologies.
