Responsibilities
  • Evaluates assessments, documentation, photos and care plans to ensure appropriate treatment and adherence to evidence-based standards. 

  • Provides real time virtual consultation with clinicians for complex patients to improve patient care and outcomes.  

  • Conducts remote chart and wound image reviews for high-risk patients with wounds and provides clinical recommendations to support wound healing and improve patient outcomes. 

  • Recommends evidence-based wound and ostomy care interventions, products and treatment approaches based on clinical characteristics and patient needs. 

  • Coordinates care for the prevention and management of complications. 

  • Utilizes advanced wound experience, clinical judgement, and critical thinking to assist clinicians and leaders in developing individualized care plans for complex wound or ostomy patients. 

  • Serves as a clinical resource and subject matter expert for care center staff, clinicians, and leaders regarding wound and ostomy care best practices. 

  • Collaborates with internal and external care providers to ensure appropriate clinical care aimed at improving outcomes. 

  • Maintains current knowledge of wound care advances, standards, and best practices. 

  • Solicits and provides feedback on educational offerings and tools/ resources related to wounds and ostomies for clinicians and leaders. 


Qualifications
  • Current, unencumbered state license to practice as a registered nurse.  

  • Board certified with the Wound Ostomy, Continence Nurses Society, The American Board of Wound Management or has completed an advanced practice nurse program (WOCN, CWS, MSN, APRN).  

  • Three years of experience working as a registered nurse performing wound care in a clinical setting. One year of home health care experience preferred.  

Preferred  

  • Knowledge of home health regulations, reimbursement models and quality programs. 

  • Experience providing remote clinical consultation using virtual technology platforms. 

  • Strong communication, collaboration, and clinical decision-making skills.
