• Conduct thorough assessments of clients’ social, emotional, and environmental challenges using evidence-based assessment tools.
• Develop and implement individualized service plans that detail goals, therapeutic interventions, and progress monitoring strategies.
• Provide one-on-one counseling and support to clients navigating various life challenges, ensuring methods adhere to trauma-informed care principles.
• Facilitate case management by collaborating with clients to connect them with community resources, including housing, food assistance, and mental health services.
• Offer crisis intervention and risk assessment when necessary, ensuring client safety and stability during emergencies.
• Work with community agencies and organizations to coordinate services effectively and advocate for clients' needs.
• Engage in proactive community outreach initiatives to raise awareness about available services and foster partnerships.
• Participate in case conferences and team meetings to discuss client progress and share insights with colleagues.

Your primary focus will be on serving school-aged children and their families who are dealing with a range of challenges including mental health issues, behavioral concerns, and socio-economic hardships. You will empower these individuals by providing tailored support, promoting resilience, and facilitating access to necessary resources that enable them to thrive in their educational and community environments.

• A master’s degree in Social Work from an accredited program is required.
• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Illinois.
• Strong understanding of social and economic justice issues, community resources, and principles of trauma-informed care.
• Exceptional communication skills and cultural competence to work effectively with diverse populations.

• Previous experience in a social service agency or educational setting is highly preferred.
• Demonstrated knowledge of evidence-based practices in mental health and social work.
• Experience developing and implementing service plans or case management strategies.
