As part of our hospice center, you will collaborate with a multidisciplinary team of healthcare professionals, including physicians, nurses, and fellow therapists, to provide comprehensive mental health support. Your role will be pivotal in driving successful patient outcomes.

• Report directly to the Social Work Manager, and evaluate, diagnose, and treat a diverse array of psychosocial issues utilizing your clinical social work skills to assist patients and their families in overcoming challenges related to mental or physical illnesses.
• Adhere strictly to the professional standards established by the National Association of Social Workers (NASW) Code of Ethics, ensuring the highest levels of integrity in practice.
• Develop bio-psychosocial evaluations and treatment plans that identify barriers to care, enhance patient health status, and optimize resource utilization.
• Provide insightful consultation to medical and support staff regarding clients' backgrounds and psychosocial needs, fostering a coordinated care approach that prioritizes excellent patient experiences.
• Implement individual and group therapy modalities, continuously assessing treatment progress to maximize positive patient outcomes and informed healthcare decision-making.
• Document social work activities in patients' medical records in accordance with departmental standards, ensuring comprehensive care records.
• Educate patients and families on understanding, accepting, and adhering to therapeutic and medical recommendations to support optimal functioning.
• Advocate for patients and their families by connecting them with community resources while emphasizing autonomy and self-determination.
• Collect and maintain case-related statistics and reports that reflect departmental standards and contribute to organizational knowledge.
• Stay current on available community resources, leveraging them as needed to enhance patient care.
• Engage in ongoing professional development through educational activities relevant to your clinical responsibilities.
• Comply with department policies to maintain a safe and legally compliant care environment.
• Utilize analytical and therapeutic skills effectively to inform treatment strategies and assess patient needs.

The ideal candidate will work closely with a diverse patient population, including individuals at various stages of life and experiences, particularly those facing end-of-life challenges within the hospice context. Your compassionate approach will help individuals and families navigate complex emotional landscapes while facilitating healing and connection at critical life junctures.

• A Master’s Degree in Social Work from an accredited institution, along with current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Kansas.
• Proven clinical knowledge and skills to serve patients effectively, especially regarding aging processes and cultural sensitivity throughout the care continuum.
• 1+ years of clinical experience, preferably in a healthcare or mental health setting with a demographic focus relevant to the communities served.
• A thorough understanding of integrating therapeutic practices that consider the unique challenges faced by individuals in hospice care.

• Experience working with hospice populations or individuals facing grief, loss, and complex life transitions.
• Skills in utilizing evidence-based therapeutic modalities and familiarity with community resource networks.
• Background in multidisciplinary team collaboration to enhance integrated care approaches.
