• Conduct thorough assessments and evaluations to identify clients' mental health concerns, particularly in a hospice setting where individuals may experience heightened anxiety and grief.
• Utilize evidence-based therapies tailored to client needs, ensuring your approaches align with best practices for end-of-life care.
• Facilitate therapeutic goal-setting sessions with clients to monitor and evaluate their mental health progress, providing consistent updates to family members where appropriate.
• Deliver teletherapy options, ensuring patients who face geographical or physical challenges can still receive the necessary support.
• Create and maintain accurate documentation of all client interactions, treatment plans, and assessments to meet ethical, legal, and regulatory standards.
• Engage in continuous professional development through training and conferences to stay current with emerging practices in hospice care and mental health.

You will primarily work with patients in hospice care, experiencing a wide range of emotional responses as they and their families navigate end-of-life issues. Your expertise in addressing grief, anxiety, and depression will be pivotal in providing support, ensuring that both patients and their families feel heard, respected, and understood during this critical phase of life.

• A master’s degree in Social Work from an accredited program.
• Current Licensed Clinical Social Worker (LCSW) designation with no disciplinary actions against your license.
• Proficient clinical skills in assessment, diagnosis, and treatment planning, with a specific focus on hospice and palliative care.
• Exceptional communication and interpersonal skills, with proven ability to engage a diverse client population effectively.
• Proficient in using telehealth platforms and comfortable adapting to various digital tools.

• Previous experience in hospice, palliative care, or a related field is highly desirable.
