• Provide proactive mental health care by assisting primary care providers in identifying, treating, and managing patients with mental health and psychosocial issues.
• Conduct comprehensive patient intakes that include diagnostic assessments and functional evaluations to inform treatment plans.
• Deliver focused interventions for clients requiring immediate mental health services, customizing approaches based on individual needs.
• Consult with primary care providers to determine appropriate referrals to specialty mental health services when necessary.
• Initiate follow-up communications with patients to monitor their progress and adjust treatment strategies accordingly.
• Develop and implement relapse prevention plans, enabling clients to maintain stability and manage their mental health effectively.
• Identify “at risk” patients and proactively create intervention plans to mitigate worsening conditions.
• Coordinate with other healthcare providers to ensure comprehensive behavioral health care delivery.
• Maintain detailed documentation of patient progress, treatment updates, and diagnostic information in a secure electronic health record system.
• Collaborate with healthcare teams to establish comprehensive treatment plans, ensuring all providers are aware of patient needs and advancements.
• Empower patients with education and self-management tools that facilitate active participation in their recovery and treatment.
• Serve as a resource for clinic management by providing insights and feedback on behavioral health services and program evaluations.
• Support patients in adhering to medical recommendations; offer strategies for coping with any medication-related side effects.
• Adhere to the professional Code of Ethical Conduct, ensuring the highest standards of practice.
• Perform additional duties as assigned to enhance the organization's therapeutic environment.

• A minimum of a Master's Degree in Clinical Counseling or a related field is required.
• Current licensure as a Licensed Clinical Professional Counselor (LCPC) in the state of Illinois.
• In-depth knowledge of mental health assessment, treatment planning, and therapy modalities, including family therapy, group therapy, and child psychotherapy.
• Proficiency in the DSM-IV diagnoses and familiarity with psychotherapeutic techniques are essential.
• Ability to evaluate patient needs effectively and adapt treatment plans accordingly.
• Strong interpersonal, communication, and teamwork skills to foster positive relationships with clients and healthcare professionals.
• Competence in managing clinical records and generating essential reports.

• Experience in a telehealth or remote counseling environment is preferred but not required.
• Bilingual proficiency in English and Spanish is highly valued to better serve our diverse client base.
• Previous experience working within a Federally Qualified Health Center (FQHC) is advantageous.

We offer a competitive salary range of USD 87,000 to 105,000 annually, depending on experience and qualifications.
