The LCPC position at Arrowhead is fully remote, allowing for flexibility while serving our patient population. • Conduct comprehensive patient intakes focusing on diagnostic and functional evaluations, delivering insights and recommendations to the primary care provider.
• Provide brief, goal-oriented interventions designed to address the immediate mental health needs of patients seeking care.
• Work within a collaborative primary care team, assisting in the management and treatment of mental health and psychosocial issues.
• Monitor patients' progress and make necessary adjustments to treatment approaches, enhancing continuity of care.
• Develop and implement relapse prevention plans to aid patients in maintaining stability in their emotional and mental health.
• Conduct outreach and follow-up communications to ensure patients are achieving their treatment goals.
• Document clinical notes and maintain accurate patient charts to reflect treatment progress compliant with regulatory standards.
• Educate patients on self-management strategies and healthy lifestyle choices to empower their recovery journey.
• Consult with clinic management team regarding program evaluation and behavioral health services.
• Adhere strictly to ethical guidelines and maintain professional standards throughout patient interactions.

As a counselor, you will engage with patients across different demographics, addressing conditions such as anxiety, depression, trauma, and relationships issues. • Master's Degree in Clinical Counseling or a related field.
• Current and valid Licensed Clinical Professional Counselor (LCPC) license in the state of Illinois.
• Strong knowledge of mental health evaluation, treatment planning, family therapy systems, and behavioral therapy techniques.
• Capacity to assess patient needs and collaborate with both clinical and non-clinical teams effectively.

• Experience working within a primary care setting or a Federally Qualified Health Center (FQHC) is a plus.
• Familiarity with community resources relevant to mental health and patient care.
• Bi-lingual proficiency in English and Spanish is preferred, enhancing our ability to serve a diverse patient base.
