Job description

Multistate Compact RN License required, reside in:
Colorado, Georgia, Indiana, Kentucky, Mississippi, Ohio, Pennsylvania, Virginia, Washington, Louisiana, Iowa

We are seeking experienced (3-5 years) Registered Nurses (RNs) to serve as Care Managers in a fully remote capacity. This role involves coordinating comprehensive, member-centered care for a diverse population with complex medical and social needs.

Various specialties needed- pediatrics, trauma, ICU experience.

Education & Experience Required:

  • Nursing Diploma or Associate Degree in Nursing
  • Active Multistate Compact RN License
  • Minimum of 3 years clinical nursing experience (acute, post-acute, or community settings)
  • MS Office Suite

Preferred:

  • Bachelor of Science in Nursing (BSN)
  • At least 1 year of case management experience in managed care
  • Experience delivering care via telephone and digital platforms

Key Responsibilities

  • Lead coordination of a multidisciplinary care team.
  • Manage care using the full case management process:
    • Assessment
    • Care planning
    • Implementation
    • Monitoring
    • Evaluation
  • Serve as the primary point of contact via phone, email, text, and digital platforms
  • Conduct comprehensive assessments of medical conditions, Psychosocial needs, Cultural preferences and Support systems
  • Develop and implement individualized care plans in collaboration with Members and caregivers, Healthcare providers, Community resources
  • Coordinate services such as Community programs, Behavioral and mental health support, Substance abuse services and financial assistance
  • Provide patient education Medication adherence, Disease management, Nutrition, Health literacy
  • Support care transitions (e.g., discharge planning, facility transfers)
